HGV, LGV & PCV/Driving/Taxi £150.00 (Patient should have an up-to-date copy of an eye test, bring forms, and a urine sample.) Subject to appointment availability. 
Adoption & Fostering  Normally paid by agency.  Subject to appointment availability. 
Request for report from insurers  £90 to complete form. £150 with examination.  6-8 Weeks. 

Private consultation (Non-Registered/Private Prescription)

£20.00 per 10 minute consult with GP.  N/A.

 

 

Private Letters/Reports

Patients request letters from their GP for a number of reasons and our doctors are frequently asked to provide private letters or reports for third parties. 

It is important when you bring your request that you bring:

  1. The correct fee in cash for the request you require. (We are not able to accept bank transfers or cheque).
  2.  An outline of what you require in the letter, or the report that you require completed.
  3. To whom the letter should be addressed. 

Request's can take up to 6-8 weeks. This is private work for the GP, and will be completed at their own discretion, and as a statement of fact. 

Reports with Medical Assessment

Some reports may require an appointment to be completed, as the doctor may need to assess you.

These can include; taxi medicals, DVLA medicals, adoption/fostering medicals. 

We will arrange this with yourself once we recieve the paperwork. 

Medical Records (Subject Access Request)

To request a full set of your medical records we ask that you attend the practice reception desk and complete our medical record request form. 

Once this information is filled out, we will action this to our secretary team to be printed. This can take up to 30 days.

You may request a set of records free of charge, once a year. If you were to request subsequent copies, this will incur a fee.
