What do I need to register?
To register with Grange Medical Practice, we will require you fill out the registration Form, and the appropriate questionnaire. As well as providing the following information:
1. We will require your NHS number, you may need to contact your previous GP practice to attain this information.
2. We will also require photo identification, ie. passport, driving license, etc.
3. We also request a proof of address.

For ease of transition of your registration, please bring your the prescription slip from your previous surgery when you register.

Register with the surgery
If you live within the practice area, an application to join the practice can be made by completing and signing an application form.
Our area bounded by the rivers Ely and Taff to the west and east, and Western Avenue to the north.

We may ask you for proof of identity and/or entitlement to NHS treatment.
You have the right to express a preference of practitioner. Such preference must be made in writing and will be recorded.
Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.
To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.
Linked profiles in your NHS account
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
Accountable GP
From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
