You can get your GP record by logging into your account using the NHS Wales App  or NHS website.

First, you need to register for online services and prove who you are. You can do this when you create an account.

You will need to request for online access to your full record from the GP Team, otherwise you will only have access to your allergy and medication information. 

You can request your full medical records by filling out our medical record request form, which is available at our reception desk. 

Requests for medical records can take up to 30 days.

You are able to request one free copy of your medical records per year, any further copies will incur a fee.
