New Patient Registration

If you live within the practice area, an application to join the practice can be made by giving us your signed medical card, or by completing and signing an application form.

Our area bounded by the rivers Ely and Taff to the west and east, and Western Avenue to the north.

We may ask you for proof of identity and/or entitlement to NHS treatment.  Registration is with the practice although, for purely administrative reasons, your medical card will show the name of one of the partners.  You have the right to express a preference of practitioner.  Such preference must be made in writing and will be recorded.

All newly registered patients are welcome to have a health check.  This includes a brief medical history, details of medication and advice about weight, diet, exercise and blood pressure.  Please bring a sample of urine to your appointment.
